Submitting an assignment
First, log in to Online Access and select the Claimcheck Dashboard.
Select Claimcheck on the left menu.
See an assignment's insurance company by clicking View Insurance Company Library.
Enter the name of the insurance company and make your selection.
Here, you can see details like company contact information, required forms, and more.
If we have the company's Death Claim form on file, it will populate in addition to the Irrevocable Assignment when entering an assignment.
Next, select Dashboard from the left menu.
Use the Claimcheck Calculator to calculate totals based on the value and request amounts you enter.
Fill in the Policy Value, Funeral Home Request, and Beneficiary Request.
Select who will be paying the assignment fee under Fee Paid By.
Policy Value is the known or assumed policy value.
Funeral Home Request is what you, the funeral home, is wanting to be paid to cover your bill.
Beneficiary Request is where beneficiaries can request a portion (or the remainder of the policy) to be fast-funded by Claimcheck.
Fee Paid By indicates who is paying the Claimcheck fee.
Click Calculate.
If the calculated numbers are accurate, click CREATE NEW ASSIGNMENT to continue.
Fill out the remaining policy information and click Add New Policy.
The policy information will list below.
Add another policy by repeating the steps under POLICY INFORMATION.
Edit a policy by clicking the pencil icon.
If you have multiple policies with different companies, you must add a new assignment from the beginning.
Click Next.
Fill out the decedent's information.
For Group policies, fill out additional information like HR contact details.
Click Next.
Select whether the Funeral Home or a Person is the Beneficiary on this policy, then complete the beneficiary's information.
If their address is the same as the decedent's, click Copy Address.
Click Add New Beneficiary.
Repeat the steps under Beneficiary Information to add any additional beneficiaries.
Then, click Next.
On Step 4 (Review and Finalize), you can review the assignment details.
If anything is incorrect, click Back to edit on the appropriate page.
If everything is correct, click Finalize Assignment.
You cannot make additional edits after the assignment is finalized.

Collecting signatures
Next, you will see a list of necessary Claimcheck documents you can view or download.
To start collecting signatures, download each Claimcheck document by clicking the paper icon next to each document.
The Death Claim Form may not pre-populate all required details. Please have beneficiaries complete this form entirely, sign it, and date it before returning.
Scan and Save all completed documents as PDFs.
To begin uploading your PDFs, click the Upload Documents button.
Select the Document Type from the drop-down menu, then drag and drop your file.
A green confirmation message will appear when the document uploads successfully.
Click the X to exit the Upload Documents window.
Uploaded documents will now appear in the Documents list.
View active assignments
To view all active assignments and their current status, click the View Assignments tab.
Scroll to see each assignment and status section as needed
To view an assignment in more detail, simply click the assignment.
