Insured Information in DIGicon

First Name, Middle Name, & Last Name - Name of the insured.
NOTE: While the middle name is not required, you may include it if you have a customer with a common name (e.g., John Smith, Mary Johnson,...).
Suffix & Prefix - If your customer specifically states this information, fill it out here to note that it is important to your customer.
Preferred Name - Use if the insured provides a nickname they prefer.
Gender - Required for finalizing.
Maiden Name - If applicable, add the insured's maiden name.
Insured (or spouse of) is qualifying for Medicaid (checkbox) - Checking this box will automatically mark the contract as Irrevocable, hide the Irrevocable option, and only allow for a Single Pay contract to be written. This helps the agent meet Medicaid requirements. While Single Pay is not the only option allowed by Medicaid, it serves as a reminder that only the money paid with the contract is considered exempt for Medicaid, not the entire contract amount.
Social Security Number - While this field is not required by Funeral Directors Life for preneeds, it may be required by your funeral home. It is good information to collect as it will be needed at time of at-need.
Date of Birth - This is required for finalization, as the insured's age will determine the payment options available for multi-pay and dollar-for-dollar contracts.
