The health questions that appear depend on the product you're writing. You may see fewer questions than the example above.
Virgina and North Carolina — Health questions can change depending on if the contract is graded or an annuity.
Health Information fields in DIGicon:

Section A - Must be answered by the applicant (even if the applicant is not the insured). This question asks whether the insured is considered to be terminally ill and will affect which payment options are available later in the contract.

Section B - Must be answered by the insured. If they are not present, do not answer these questions. Answering them allows the contract to be fully insured. If both are answered “No” and you have doctor’s information, the contract can be fully insured.

Previous Physician Lookup - This field will search for any physicians previously entered in DIGicon. If available, the doctor's address and phone number will auto-fill.

Personal Physician's Name - If the physician does not appear in the Lookup field, type the doctor's name using the following format: Dr. John Doe

Address/City/State/ZIP - Enter the physician's office address using the following format: 1234 Main St, City, ST 55555

Phone - Provide the physician's phone number.
